Subject: change #include "jtag.h" to <jtag/jtag.h>

Changes from the flat namespace to heirarchical one.  Instead of writing:

	#include "jtag.h"

the following form should be used.

	#include <jtag/jtag.h>

The exception is from .c files in the same directory.
